An anal fistula is a narrow passage connecting the interior of your rectum to the surrounding skin. It forms when an infection originates in a gland near your rectum, leading to the accumulation of pus. Over time, this pus can develop into a small tunnel, medically referred to as an anal fistula.

The Standards Committee for the American Society of Colon and Rectal Surgeons (ASCRS) divided anal fistula into simple and complex types based on the complexity of fistula, risk of recurrence, and risk of incontinence.

Simple fistula:

Simple fistulas are those that are Intersphincteric or low Transsphincteric involving less than 30% of the external sphincter where the risk of incontinence is low.

Complex fistula:

“Complex” fistulas included those with

  1. More than 30% sphincter muscle involvement
  2. Anterior fistulas in female patients
  3. Recurrent fistulas
  4. Fistula associated with preexisting fecal incontinence, inflammatory bowel disease, or prior radiation.
